COMPLETING THE APPLICATION: (Top)
The link to obtain the internship application is located at the beginning of this page. Once you click the link to "Items needed to begin the application process", you will find the links to the Undergraduate and Graduate Internship Applications. As part of the application process, you will complete the blue application form. Then, you will click "enter data" and the application will appear in a printable format. Print the application and then click "next page". This will take you to a PDF document (pages 1-8). You will need to fill out and print each page. Please email Dianne Wood, wood@tempest.coedu.usf.edu with any questions.
NOTE: You must READ each alert box and follow the directions in order for this process to be successful.
The directions for the on-line internship application process will be available throughout the entire process. You can locate the white "Directions" button at the top of each form. After reading the directions you will be required to close that window to continue the on-line internship application process.

Page 2 - General Knowledge and FTCE Information (Top)
On the General
Knowledge portion of
the form please read the statement and
type your name.
On the FTCE portion of the form, type your
name and indicate the date you took or will be taking the FTCE.
Undergraduate interns must submit passing scores of both the Professional and Subject Area of the FTCE to the Internship Office as part of your internship grade. Graduate interns must submit official Professional and Subject Area FTCE scores to EDU106E.
Page 3 - Advisor's Eligibility Verification (Top)
Type Name, Major, Degree (BA, BS, MA, ND = non degree), and USF ID number.
GPAs for professional and specialization may be obtained from your personal degree audit for Undergraduates only.
Graduate students submit USF Cumulative and Specialization GPA's.
Undergraduate Degree Audit (Top)
You can view and print your Undergraduate Degree Audit from FACTS via the Internet.
Visit the FACTS website at http://www.facts.org
You can also access your Undergraduate Degree Audit from the USF homepage then click on OASIS.
List the following course(s):

Undergraduate Interns: Each Undergraduate program requires students to successfully complete specific coursework
before interning and any course(s) listed other than Sr. Seminar or EEX
4070 must be approved and an approved College of Education petition
submitted with application). See Internship Office for information and
petition.
Graduate
Interns: Your faculty advisor must recommend approval of any additional
coursework to be taken during or immediately after internship. (Some courses
have been pre-approved by the college for this purpose.) In instances where
this pre-approval has not been granted, a written request and an academic
rationale must be sent to the COEDU Graduate Coordinator and Associate Dean
for Educator Preparation by the student’s
advisor. These requests will be reviewed by the Associate Dean for Educator
Preparation. In these instances, if approval is denied, you may be unable
to intern during the semester for which application was made.
C. Courses to be taken after internship. NOTE: Undergraduate interns-program advisor must approve courses listed. Undergraduate Interns see Internship Office for information and petition. Graduate interns-Program advisor must recommend courses.
Advisor will sign the application in the appropriate places after it has been submitted.
Page 4 - Disclosure of Arrest Record (Top)
Complete name, social security number, and intended degree at top of page.
Read statement regarding School Districts right to know about arrests.
Please check either "yes" or "no".
If yes has been checked off please include two copies of arrest records, court distribution of case and final outcome as well as a personal statement of events.
Do not send originals of information. Originals must be kept for future reference. This information will be sent to the county as part of your application.

Indicate degree you are seeking. Bachelor's will receive a BS if you have not taken two semesters of college foreign language and required classes.

Address while interning - where you will be living while you complete your internship. If any of these addresses change, please, notify the Internship Office in writing.
E-Mail address - will be used for mass notices, please check your e-mail often.
You must register for the section that corresponds to the county in which you will complete your internship. If you have any questions please see your departmental advisor.
Please check all levels of assignment that may pertain to your major. (Elementary & Secondary Education majors only
If you have checked public, list only the county preference. Do not list individual schools in this area. Do not list geographic location (northwest, east). Counties make the placements and placements are FINAL.
If you checked Non-Public School, list the school choice and contact the Internship Office for further information on placement in a non-public school.

Must be handwritten.
Should represent your best composition and handwriting.

REQUEST A TRANSCRIPT FROM USF: (Top)
Transcripts can be obtained from the Office of the Registrar. Transcript request forms are available at Office of the Registrar. Please allow 3-5 working days for processing your request.
Undergraduate Interns: Open your transcript and make a photocopy. Then, attach the original to your original application and attach the photocopies of your USF transcripts to all copies of your application.
Graduate Interns: Must submit two official USF transcripts.
NOTE: The Internship Office request that you order USF official transcripts after current semester grades are posted (i.e. for Spring applications wait for current Spring grades to post before ordering your transcript).
The county administrators have asked that they be made aware of any special circumstances that may affect your internship placement. Examples are a disability accommodation or transportation needs. If you have a particular need that the county should be made aware of, please complete the Special Placement Information box on page 5 of the application. You are not permitted to request a particular school, therefore, this form is not to be used for that purpose. The Internship Office reserves the right to deny any requests that are deemed inappropriate.
INTERNSHIP ORIENTATION and TRAINING SESSIONS: (Top)
Fall 2010 semester begins on Monday, August 30, 2010. Internship Orientation and Code of Ethics training for the Fall 2010 semester will be held that week. Attendance is required and will be monitored. Orientation and training are considered to be part of your internship experience. If you are unable to attend the Orientation or Training Session, you must attend a make-up on the Tampa campus. Failure to attend orientation may result in you being withdrawn from your school placement. Information regarding Placement, Orientation and Training will be mailed to your home no later than mid July 2010.
1. You may apply to do your internship in the following counties unless your program has specifications listed in their programmatic material and/or the University Catalog. Desoto, Hardee, Hernando, Hillsborough, Highlands, Manatee, Pasco, Pinellas, Polk, Sarasota.
* Citrus (Secondary Education majors must have pre-approval through the Secondary Education Department to intern in Citrus County and Childhood Education majors must contact the Department Chair in Childhood Education and Literacy Studies (EDU 202) for pre-approval to intern in Citrus County )
* Sumter (Secondary Education majors must have pre-approval through the Secondary Education Department to intern in Sumter County and Childhood Education majors must contact the Department Chair in Childhood Education and Literacy Studies (EDU 202) for pre-approval to intern in Sumter County ).
2. You may intern in a non-public school rather than a public school IF the school meets state accreditation criteria, IF your department and program agree and, IF the school accepts you. You must indicate the specific school you want at the time your internship application is submitted. Prior to selecting a non-public school, confirm with the internship office that the school is accredited. You must provide the internship office with documentation that your request to intern in a non-public school has been approved by your department and that you have met with the school and they can provide an appropriate placement (A letter from the principal will be required).
3. Students should not intern in a school that you have recently attended.
4. Do NOT contact schools or county offices to check on your application. The county offices make all intern assignments and then sends notification to the Office of Student Academic Services in the College of Education. You will receive written notification from the Office of Student Academic Services regarding your placement no later than July 2010.
5. You must adhere strictly to the time schedule and calendar of your assigned school.
6. Undergraduate Interns: Each program requires students to successfully complete specific course work prior to interning.
a. Elementary and Early Childhood programs require completion of all professional education and specialization course work except for HLP 4722 and Senior Seminar.
b. Physical Education program requires completion of all professional and Specialization course work except for PET 3252.
c. Special Education programs require completion of all professional education and specialization coursework except for senior seminar.
d. Secondary Education programs require completion of the professional course sequence except for EEX 4070 and EDF 4430 and a minimum of two-thirds of the specialization coursework.
e. Elementary, Early Childhood, and Physical Education programs require a combined grade point average of 2.50 in professional education and specialization coursework as well as an overall USF gpa of 2.50
f. Special Education programs require an overall USF gpa of 2.50.
g. Secondary Education programs require a minimum gpa of 2.50 in professional education and specialization coursework and an overall USF gpa of 2.50
h. Student must earn a minimum “C-“ grade or “S” in their required major courses.
7. You must be willing to assume the responsibility of transportation to the school where you are assigned. (See consent form)
By the time you begin your internship, you must have met the following eligibility criteria:
Admission to the College of Education.
Completion of General Education requirements.
Completion of the CLAS.
Completion of the General Knowledge Test (pass all sections).
Completion of an application for internship by the due date.
Undergraduate Interns Only: Completion of the professional education sequence except for EEX 4070 (Integrating Exceptional Students in Regular Classes) in education courses (a College of Education petition must be submitted for approval to take any other courses); a minimum of two-thirds of specialization; no education course work with less than a "C" grade; a minimum of a 2.5 grade point average in each area; or a USF 2.5 grade point average.
Completion of other requirements prescribed by the applicable program.
