Please be advised that the Hillsborough County School District has recently requested that each intern in the district MUST complete the Criminal Record Information Form.

After printing the Criminal Record Information Form above, please print your name and social security number at the top of the form and answer all questions. Remember to sign and date the form at the bottom and include this form with your Internship Application Packet.
