On-Line Annual Report

Faculty members should complete and submit their on-line annual report on the FAIR system, as well as the following:

• Modules 1-8
• The on-line University Annual Faculty Review Summary Form
• Documentation file (as required by departmental guidelines)

Please refer to “Completing the Annual Report Using the FAIR System” for instructions on how to complete each module.  The instructions are also on-line.

Non-Teaching & Research (Non T & R) Faculty

Please begin your annual report process by going to http://isis.fastmail.usf.edu/FAIR/login.asp and following the instructions for “ANNUAL REPORTS.”

The annual report is comprised of nine (9) modules.  You will not have to complete all modules – only those which are relevant to your assignment.  For example:  If you have a 75% research and 25% administrative assignment, then you only have to complete the modules corresponding to those assignments (research and administration).

All regional non-teaching and research faculty will follow the same on-line process outlined above.  For further information, please refer to pages 22-23 of the ICAR document (Inter-Campus Academic Relations) dated January 20, 2005.
